Online One on One Fitness Training
Welcome to my online training program.
Thank you for choosing me as your coach.I am so excited to work with you.
It promises to be the start of a happier and healthier YOU.
You will receive 6 exercise videos each month plus a meal plan.
In order for us to get started I need to get to know you a little bit better and find out what your goals are and work out a strategic plan for us to follow.
So please fill out these questions and mail them back to me a.s.a.p. so that I can work on a meal plan and exercises plan for you.
1. Where will you be training?
2. What time of day will you be training?
3. When last did you follow any exercise program? and what type of exercises did you do?
4. How fit/unfit are you on a scale of 1 to 10.
1 being very fit and 10 being very unfit.?
5.Do you have any physical challenges or injuries that I should know about?
6.Are you allergic to anything?
7. Any food preferences?
8. Any foods you really dislike?
WHAT I WILL NEED FROM YOU TO START, PLUS EVERY 6 WEEKS
1. Measurements taken in cm’s ( see attached measurement chart)
2. 3 Progress pictures taken in swimwear or underwear ( back,side and front)
3. In-Body scan if possible ( can be done at any medical professional or gym)
4. Monthly report card (see attachment )
MONTHLY REPORT CARD should be filled out every 4- 6 weeks:
1. What was your goal for this month? and why did you reach it / not reach it?
2. What was your highlight this month or any personal best ?
3. What was your biggest struggle this month?
Send this to me every month including your measurements and updated progress pictures.
